Donation Agreement

I AGREED TO THE TOWN OF COTTESLOE'S COMMUNITY DONATIONS AGREEMENT *

I acknowledge that I have read and understand the Town of Cottesloe's Donations Policy and the donation assessment criteria. If this application for donation is approved, I agree, on behalf of the organisation to provide the Town of Cottesloe with a tax invoice for the amount granted; present the Town of Cottesloe with a report on the project, including a financial report of the donation expenditure, copies of material which feature the Town’s logo and a brief assessment of the project’s success (only applicable for donations over $500); and to notify Council immediately, if the donation amount granted is intended to be used differently to what was listed in the application form.
