Settlement Enquiry Form - Town of Cottesloe

Complete this form if you are taking over a food business that is currently operating, it is recommended that your settlement agent requests a written report from the Town's Environmental Health Services. A report advises of the last food safety inspection, any outstanding work orders and fees and charges that are applicable to this business

Food Act Notification Registration Form - Town of Cottesloe

Complete this form if you propose to operate a Food Business where food is stored, sold, prepared or manufactured. All food businesses are required to be registered, irrespective of the quantity or nature of food sold.

Application to Conduct Outdoor Eating Facility

Complete this form if your Business proposes to sell food outdoors on Council property. Please ensure you submit the completed form with two sets of plans & specifications of the proposed eating area.
